Under the scope of the Law No. 6698 on the Protection of Personal Data 

Data Subject Information Request and Application Form

Pursuant to Article 11 of the Law No. 6698 on the Protection of Personal Data, data subjects, defined as the relevant individuals, may submit various requests to the data controllers in writing regarding the processing of their personal data. ELESTAS ELECTRICITY GENERATION INC. ("Company")is the data controller pursuant to the first paragraph of Article 13 of the relevant Law.

Please refer to the Personal Data Protection Law (“PDPL)In accordance with the Personal Data Protection Law ("PDPL"), please fill out the application form below clearly and completely and send it to us in order for your request to be processed.

We will respond to your application as soon as possible, and no later than thirty days, depending on the nature of the request. If the information and documents you provide are incomplete or unclear, we will contact you to clarify your application. 

Our Company reserves the right to request additional documents and information for identity and authorization verification in order to eliminate legal risks arising from unlawful and unfair data sharing, and particularly to ensure the security of your personal data. In case the information provided in the form is incorrect or outdated, or if an unauthorized application is made, our Company will not be held liable for any requests arising from incorrect information or unauthorized applications. All responsibility for unlawful, misleading, or erroneous applications lies with you. In accordance with the Communique on Procedures and Principles for Applications to the Data Controller, if the response to your application exceeds 10 pages, a processing fee of 1 Turkish Lira will be charged for each page after the 10th page. 

In this context, "written" applications to our Company must be made by printing out this form;

• By making a personal application,
• Through a notary, 
• The application must be signed with a "secure electronic signature" as defined in the Electronic Signature Law No. 5070 and sent to our Company's registered electronic mail address at elestas@hs01.kep.tr.

• I would like it to be sent to my email address. (If you choose the email method, we will be able to respond to you more quickly.)

• I would like to collect it in person. (In case of collection by proxy, a notarized power of attorney or authorization document is required.)

If you are submitting an application on behalf of someone else, please send the documents proving your authorization to make the application (such as a document showing that you are the guardian or conservator of the personal data owner, power of attorney, etc.) along with your application. These documents must be issued or approved by the relevant authorities to be considered valid.
